Led by the Indigenous Primary Care Team, this survey aims to learn about the healthcare services that members of Fort Albany, Kashechewan, Attawapiskat, Peawanuck, Moosonee and Moose Factory want in their communities. Share your input and help guide the planning of healthcare services in your community.
Elders, or those not familiar with online surveys can have a family member or friend assist in entering information for them. This survey is completely anonymous. We value and appreciate your contribution.
