Case Manager

SCO is a growing and dynamic regional First Nation organization that represents 33 Anishinaabe and Dakota Nations and more than 92,000 citizens in what is now southern Manitoba. Established in 1999 by the Chiefs, SCO is mandated as an independent organization to protect, preserve, promote, and enhance First Nation peoples’ inherent rights, languages, customs, and traditions through the application and implementation of the spirit and intent of the Treaty-making process.

OVERVIEW:

The Case Manager supports clients with intake, stabilization, and next steps, including referrals to community and health services, including working closely with Elders, Protective Support Workers, Supervisors, and external partners to ensure clients have access to appropriate care pathways such as medical, detox, shelter, addictions supports, mental-health services, or cultural healing resources. This role requires humility, compassion, non‑judgment, and a strong understanding of Indigenous worldviews, harm‑reduction philosophy, and trauma‑informed practice.

Positions Available:  1 (Full-Time, Permanent)
Compensation: $85,000 – 95,000 annual salary
Schedule: Shift work may be required (variety – days, evenings, nights, weekends)

KEY RESPONSIBILITIES:

  • Conduct intake and brief assessments
  • Develop safety and wellness plans
  • Connect clients to community, health, and cultural supports
  • Coordinate referrals and transitions
  • Maintain accurate documentation
  • Collaborate with internal team and external partners

EDUCATION AND QUALIFICATIONS:

  • Experience in case management, outreach, or community services
  • Knowledge of harm reduction and trauma-informed practice
  • Strong coordination and communication skills
  • Understanding of Indigenous communities and systemic barriers
  • Certifications: First Aid & CPR (Level C) required; Naloxone/Narcan competency required; Non‑Violent Crisis Intervention (NVCI) preferred; ASIST and/or Suicide Risk Assessment training considered strong assets

OTHER REQUIREMENTS:

  • Must provide a current and satisfactory Criminal Record Check with Vulnerable Sector Search
