Under the direction of the Home and Community Care Service Manager, the Care Manager is the primary person responsible for assisting clients and their support systems in maintaining the highest possible level of independence and functioning in the least restrictive environment within a rural community setting. The Care Manager conducts comprehensive clinical assessments, develops service plans, assigns and coordinates professional and paraprofessional services, monitors services provided, reassesses clients routinely and plans for discharge. Care Managers are responsible for implementing the above responsibilities in a cost-effective and integrated manner.
Qualifications and Requirements: A four-year BScN degree is required. Three years direct experience in the delivery and/or coordination of home-care services; demonstrated sound clinical knowledge and experience in home-care case management and/or community based home-care service delivery; and a combination of education and work experience pertaining to community based health care services may be considered. Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered.
Must have: The ability to work flexible hours; travel regularly and provide “on-call” services as required; must provide evidence of registration with the appropriate regulatory bodies(College of Nurses and Registered Nurses Association of Ontario); a valid Class “G” Driver’s License; access to a reliable, personal vehicle; $1,000,000 liability vehicle insurance and have a Standard First Aid and CPR certificate.
Wage Range: $31.11 – $40.59
Hours of Work: 32.5 hours per week
